What Specific Info do you Log

Back in March I posted a question asking about how members log their receptions. http://www.hfunderground.com/board/index.php/topic,10294.msg30202.html As I am now getting closer to being actually able to hear something, (my AFREDI SDR is currently in the hands of USPS here in Texas  ;D and I am working on getting an antenna put up) I would like to get more specifics about what exactly is logged.  My only logging to date was when I was active on the Amateur Satellites several years ago so I have a general idea. What I am interested in is things more specific to SWL.  Since I'm just beginning I'll probably be trying to find just about everything, but I think my interests will tend more toward the utility/number/oddity type stations. 

I'm trying to create a spreadsheet or maybe get really ambitious and try a database like Token mentions in above mentions thread.

Typically:

date and time for start and end of broadcast, in UTC
frequency and mode (AM, USB, etc.)
SIO or SINPO
your location (for pirates, I would not make this too specific)
your equipment, typically at least your radio and antenna
comments on the content, with the intent of showing that you actually heard the broadcast
pirates often like to hear a critique of the program material, such as whether you liked it, what parts were good or bad, and so on
